Children's Product Certificate (CPC) template: the 7 fields and the mistakes Amazon flags
CPSC requires no fixed format, only 7 elements. A copy-ready field-by-field template, plus the mismatches between CPC and test report that get documents rejected.
The problem
CPSC says no specific template has to be used, as long as the 7 required elements are present and accurate. It also publishes sample CPCs for a toy, clothing and a bassinet. Most rejections are not about format. They come from fields that disagree with the test report or the listing.
Template (one line per element):
- Product covered: name, model/SKU, brand, description detailed enough to match this product and no other.
- Safety rules certified to: each children's product safety rule by citation (e.g. 16 CFR 1250 / ASTM F963-23; 16 CFR 1303 lead paint; CPSIA ยง101 total lead; 16 CFR 1307 phthalates). List exempted rules too.
- Certifier: the US importer or domestic manufacturer: name, full mailing address, phone.
- Records contact: person keeping test records: name, full address, email, phone.
- Date and place of manufacture: at least month/year; at least city and country of final assembly.
- Date(s) and place(s) of testing: test or report dates and location. Exemption citations can go here.
- CPSC-accepted lab: name, full address, phone. Small batch manufacturers list their CPSC registration number instead.
How to fix it
- Fill each field from the test report itself, not from memory or the factory's template.
- Copy the model/SKU and product name exactly as they appear on the report and the ASIN.
- Cite every rule the report actually tested. Don't cite rules that weren't tested.
- Use your company (the importer) as certifier, not the factory or the lab.
- Check the lab against CPSC's accepted-lab search for the rules you cite.
Checklist before resubmitting
- All 7 elements present
- Identifiers identical to report and listing
- Cited rules = tested rules
- Importer is certifier
- Lab is CPSC-accepted for those rules
- In English
